The StrongMind Community Model of Care is a culturally responsive, evidence-informed framework designed to engage vulnerable men, strengthen community capability, and foster sustainable healing. It blends lived experience, professional practice, and community connection to rebuild identity, leadership, and wellbeing in men and families.
Modular Program Components
1. Healing Workshops & Immersive Interventions
StrongMind delivers structured workshops and community-based intensives that target addiction, anger, emotional regulation, and harmful behaviour patterns.
Delivered over 2–5 days or through ongoing local partnerships
Focused on healing, accountability, and reconnection
Uses practical tools and emotional pacing to support long-term change
Leadership pathways are embedded, with participants invited to co-facilitate, share insight, and build facilitation skills through supported practice
2. Individual Case Work & Behavioural Support
We provide 1-on-1 support for vulnerable men in the community, bridging the gap between program participation and real-world application.
Personalised case planning and mentoring
Relapse prevention and emotional regulation coaching
Anger management and behavioural change sessions
Goal-setting and self-awareness development
Participants may be invited into peer mentoring roles, reinforcing leadership and accountability
3. Community Forums & Collaborative Conversations
StrongMind facilitates open forums that bring together men, families, service providers, and community leaders.

Strengthens cross-sector collaboration and reduces stigma
Forums often feature participant-led segments, promoting visibility and confidence in emerging community leaders
4. SEWB Staff Upskilling & Capacity Building
To ensure sustainable outcomes, StrongMind delivers professional development for Social and Emotional Wellbeing (SEWB) staff and community workers.
Topics include trauma-informed care, motivational interviewing, and exit planning
Delivered via workshops, shadowing, and co-facilitation pathways
Builds rapport, manages resistance, and enhances local capability
Leadership cultivation is modelled throughout, with community members supported to take on facilitation roles over time
5. Post-Program Follow-Up & Continuing Support
StrongMind offers a flexible follow-up phase tailored to the needs of each community and partnering organisation. This phase reinforces learning, strengthens aftercare outcomes, and reduces the risk of relapse.
Support options include:
Telehealth Support: Weekly calls with StrongMind facilitators focused on relapse prevention, emotional regulation, and goal setting
In-Community Return Visits: Facilitators return for 2–3 days each month over 6–12 months, offering group refreshers, mentoring, and reconnection support
Hybrid Models: Combining digital check-ins with periodic in-person engagement
All formats include:
Continued coaching and accountability
Support with referrals and service reconnection
Opportunities for participants to step into peer support and leadership roles, reinforcing community ownership and healing momentum
Evaluation & Feedback Integration
StrongMind programs include structured feedback loops to ensure continuous improvement and community relevance.

Governance & Quality Assurance
StrongMind maintains internal quality assurance through facilitator training, safeguarding protocols, and regular program audits. All services are delivered in alignment with national rehabilitation standards and community-led best practices.
