StrongMind’s Community Model of Care is designed to engage vulnerable men, strengthen local support systems and create sustainable healing within communities.

The model combines lived experience, professional practice, culturally responsive support and community connection to help men rebuild identity, develop accountability, strengthen wellbeing and move toward lasting change.

Our Model

A Practical Framework for Community-Led Healing

StrongMind’s Community Model of Care is built for men, families, frontline services and communities dealing with addiction, anger, trauma, disconnection and harmful behaviour patterns.

Rather than relying on one-off support, the model creates a pathway for deeper engagement, practical behaviour change, ongoing follow-up and local capability building.

The framework brings together healing workshops, individual support, community forums, staff upskilling, aftercare and feedback processes to support change that continues beyond the program itself.

At its core, the model is about helping men take responsibility, understand their patterns, reconnect with purpose and build safer ways of living — while also strengthening the people, services and communities around them.

Modular Program Components

1. Healing Workshops & Immersive Interventions

StrongMind delivers structured workshops and community-based intensives focused on addiction, anger, emotional regulation, trauma and harmful behaviour patterns.

These sessions can be delivered over 2 to 5 days, or through longer-term local partnerships depending on the needs of the community.

Workshops focus on healing, accountability, reconnection and practical tools that support long-term behaviour change. Emotional pacing is used throughout to create a safer environment for men to reflect, engage and take ownership.

Leadership pathways can also be built into the program, with participants invited to share insight, co-facilitate, support others and develop facilitation skills through supported practice.

2. Individual Case Work & Behavioural Support

StrongMind provides one-to-one support for vulnerable men, helping bridge the gap between program participation and real-world change.

This support may include personalised case planning, mentoring, relapse prevention, emotional regulation coaching, anger management, behavioural change sessions, goal-setting and self-awareness development.

Where appropriate, participants may also be invited into peer mentoring roles, helping reinforce leadership, accountability and community ownership.

3. Community Forums & Collaborative Conversations

StrongMind facilitates open community forums that bring together men, families, service providers and community leaders.

These forums create space for shared lived experience, honest conversation, resource sharing and practical strategy development.

They help reduce stigma, strengthen cross-sector collaboration and build confidence across the community. Forums may also include participant-led segments, allowing emerging community leaders to be seen, heard and supported.

4. SEWB Staff Upskilling & Capacity Building

To support sustainable outcomes, StrongMind delivers professional development for Social and Emotional Wellbeing teams, frontline workers and community staff.

Training can cover trauma-informed care, motivational interviewing, rapport building, managing resistance, exit planning, emotional safety and behaviour change support.

This can be delivered through workshops, shadowing, co-facilitation pathways and practical team development. Over time, community members and local workers can be supported to step into stronger facilitation and leadership roles.

5. Post-Program Follow-Up & Continuing Support

StrongMind offers flexible follow-up support tailored to the needs of each community and partnering organisation.

This phase reinforces learning, strengthens aftercare outcomes and helps reduce the risk of relapse or disengagement.

Support options may include weekly telehealth check-ins, in-community return visits, group refreshers, mentoring, reconnection support or hybrid models combining digital and face-to-face support.

All follow-up options are designed to provide continued coaching, accountability, referral support and opportunities for participants to step into peer support or leadership roles.

Evaluation, Feedback & Quality Assurance

StrongMind programs include structured feedback loops to support continuous improvement, community relevance and stronger outcomes.

Evaluation can include participant feedback forms, facilitator debriefs, reflective practice sessions, community partner check-ins and collaborative program reviews.

Where required, StrongMind can also provide post-program reporting that summarises key outcomes, participant feedback, attendance, engagement levels, service connections and relapse prevention progress.

StrongMind maintains internal quality assurance through facilitator training, safeguarding protocols, reflective practice and regular program review.

All services are delivered with a focus on safety, cultural responsiveness, community-led practice and practical support for long-term healing.
